Dear Customer,

This form MUST be completed to support your compensation claim.

  • Please Note: Only 1 Form per booking is required, NOT 1 Form per person on the booking.

  • Do NOT complete multiple Forms, by doing this you will delay your case. Our system removes the Oldest Form raised by you and keeps your latest Form (as we assume you are providing new details that we need to investigate your claim).

  • Please complete all the fields required with the correct details to support your claim.

  • Failure to provide all the correct information will prevent flyadeal from investigating your case.

  • Any claims relating to Lost, Delayed, Damaged Baggage MUST be supported with a Property Irregularity Report (PIR) form, which is completed by our Representative in the baggage hall at your arrival airport.

  • We will NOT be able to accept any baggage claims without this PIR form.

  • If you feel you need to provide further information to support your claim you can add comments to the Text Box and/or attach documents / pictures as required.

Any compensation to be paid by flyadeal will be via a bank transfer and therefore we require your Bank Details - IBAN, Account Holder Name, Bank Name.

Please complete this detail carefully as mistakes can result in payments being made to a different person, flyadeal will not be responsible for this error.
